At Teddington School, the health and wellbeing of our students is a top priority. If your child has a medical condition or requires medication during the school day, please complete the Healthcare Plan and Medication Consent Form below.
The forms will be reviewed and logged by the school office to help us ensure your child receives the appropriate care and support while at school. If your child does not have any medical needs, no action is required.
Healthcare Plan
Please provide as much detail as possible when completing the Healthcare Plan to ensure we have all the relevant information to support your child effectively. If your child has a hospital-issued healthcare plan, please send a copy to the school office once you have completed our form.
Medication Consent
The Medication Consent Form must be completed if your child needs to take prescribed medication during the school day, or while attending school activities including trips and visits.
Please ensure the required medication is brought to school as soon as possible. Your child can take it directly to Student Services, or a parent/guardian may hand it in at Reception.
A Healthcare Plan must be completed with the Medication Consent Form.

Antibiotics
If your child is required to take prescribed antibiotics during the school day, please complete the consent form provided below.
Students should bring the antibiotic into school and hand it in to Student Services at the beginning of the day. Alternatively, a parent/guardian may drop the medication at Reception.
Please note that students are not permitted to carry any medication on them during the school day.
If necessary, your child may collect their medication from Student Services at 15:00 to take home and should return it the following morning.

Administration of Medication During the School Day
If your child requires paracetamol, ibuprofen, or an antihistamine during the school day, a member of staff will contact you to request your consent before any medication is administered.
The only exception to this is if you have provided the medication along with details of a health condition and a completed consent form, in which case the medication will be administered as agreed.
